All Scottish Fold cats carry a genetic mutation causing feline osteochondrodysplasia (FOCD), a condition that affects cartilage and bone formation throughout the body and leads to progressive arthritis. This means the breed's defining folded ears are a symptom of a systemic skeletal disorder that causes pain, with severity ranging from subtle to requiring euthanasia.

The mutation affects every cat

The folded ears are not a cosmetic quirk but a sign of a genetic defect. The breed is defined by an autosomal dominant mutation in the Trpv4 gene, specifically a V342F substitution, which disrupts calcium channels in cartilage cells. Because the gene is dominant, any cat with folded ears possesses the mutation, and research indicates that all Scottish Folds develop some degree of the condition. The cartilage defect causes the ears to fold forward and down, but it also affects the tail, making it short and stiff, and the joints, leading to abnormal bone growth.

Severity varies but pain is universal

While the condition is present in every cat, the clinical expression varies significantly. Heterozygous cats, which have one copy of the mutation, develop a progressive form of arthritis that can begin at a young age. Homozygous cats, possessing two copies of the mutation, suffer from a much more severe and rapid onset of the disease. In some cases, the skeletal abnormalities become so debilitating that the cat requires euthanasia while still young. A 2021 study of radiographs suggested that the severity in heterozygous cats can be subtle and variable, potentially influenced by other genes or environmental factors, but the underlying pathology remains.

Genetics dictate breeding outcomes

The inheritance pattern is complex and drives the ethical controversy surrounding the breed. A cat with folded ears may be heterozygous (Fd fd) or homozygous (Fd Fd). Breeding two folded-eared cats together creates a 75% chance of producing folded offspring, including 25% that are homozygous. These homozygous kittens are at high risk for severe, crippling deformities. Consequently, ethical breeding programs mate folded cats only with straight-eared cats (fd fd), which yields a 50% chance of folded offspring and 50% straight offspring, avoiding the homozygous state. However, even these "safe" pairings do not eliminate the risk of FOCD, as heterozygous cats still develop the condition.

Health complications beyond the ears

Owners should be aware that the health issues extend far beyond the ears. The condition causes exostoses, or bone spurs, to form near the tarsal and metatarsal joints, leading to lameness and pain. Treatment options include palliative radiation or surgery to excise bone spurs or fuse joints, but these are management strategies rather than cures. Additionally, the breed has a higher prevalence of polycystic kidney disease (PKD) and hypertrophic cardiomyopathy compared to other breeds. A study in Japan found that 54% of tested Scottish Folds carried the PKD1 mutation, and other studies have linked specific genes to heart disease in the population.

Legal and registry restrictions

Due to the welfare concerns associated with FOCD, the breeding of Scottish Folds is prohibited in several countries, including the Netherlands, Austria, Belgium, Norway, Scotland, and Sweden. Major registries such as the GCCF and FIFe do not recognize the breed, having withdrawn registration in the early 1970s due to concerns about crippling deformities and ear infections. While the Cat Fanciers' Association (CFA) granted championship status in 1978, the breed remains controversial. Some registries in New Zealand have recommended mandatory X-rays and periodic veterinary exams for breeding cats, but the consensus among many researchers is that the only solution to the suffering caused by this mutation is the cessation of breeding the breed entirely.

What the name actually means

The breed was originally called "Lop-ears" after the rabbit, but the name was changed to Scottish Fold in 1966. The long-haired version is variably known as Highland Fold, Scottish Fold Longhair, or Coupari. Despite the name, the "Scottish Straight" is a distinct category within the breed; these are cats that do not develop the fold but may carry the gene. They are often used as outcrosses to produce folded kittens, but they themselves are genetically carriers of the mutation and can develop the condition later in life.

The Scottish Fold is a breed where the aesthetic appeal of the folded ears is inextricably linked to a painful genetic disorder. While the cat's rounded head, large eyes, and "sweet expression" are popular, the reality is that every individual of the breed carries a mutation that compromises their skeletal health. Prospective owners must weigh the desire for this specific look against the certainty of potential lifelong pain and the ethical implications of supporting a breed with such significant welfare concerns.
