Why deep-chested breeds are at risk
Breed matters more than size, though large, deep-chested dogs are disproportionately affected. The anatomical shape of the thorax, specifically a high depth-to-width ratio, significantly increases the likelihood of the stomach rotating. Great Danes are the most at risk, with a lifetime risk estimated between 37% and 42.4%, making them 10 times more likely to develop GDV than other breeds.
Other high-risk breeds include the Weimaraner, St. Bernard, Irish Setter, Irish Wolfhound, and Standard Poodle. The exact sequence of events is not fully understood, but the immediate prerequisite is a dysfunction of the esophageal sphincter and an obstruction of the pylorus. The stomach twists clockwise in 90% of cases, typically rotating between 180 and 270 degrees, though rotations up to 360 degrees have been recorded.
Once the twist exceeds 180 degrees, the esophagus is closed off, trapping gas inside. This creates a pressure cooker effect that compresses the portal vein, reducing blood flow to the liver and preventing the removal of toxins and bacteria from the blood. Recognizing the symptoms
Owners often mistake the signs for simple indigestion, but the distress is extreme. A dog will stand uncomfortably, appear depressed, and exhibit nonproductive retching, attempting to vomit without producing anything. The abdomen becomes firm and distended, a condition known as tympany, and the dog may struggle to breathe.
Systemic signs include weakness, hypersalivation, and pale mucous membranes with poor capillary refill. Foods with high levels of added oils or fats may delay gastric emptying, contributing to the problem. Conversely, obesity appears to be protective compared to underweight dogs. Other medical conditions like inflammatory bowel disease, gastrointestinal disease, or nasal mite infections are also associated with higher incidence rates.
How it is treated and prevented
Treatment requires immediate stabilization of blood pressure and perfusion before any surgical intervention. This involves intravenous fluid therapy, antiarrhythmic drugs, and analgesia. The stomach must be decompressed, either via a catheter or an orogastric tube, before the dog is taken to surgery.
Surgery involves rotating the stomach back to its normal position and performing a gastropexy, which sutures the stomach wall to the abdominal wall. This prevents recurrence, which occurs in up to 80% of dogs treated without the procedure. Prophylactic gastropexies are recommended for high-risk breeds or dogs with a family history of the condition.
Outcomes and prognosis
Time is the most critical factor. A delay in treatment greater than six hours significantly worsens the prognosis. While historical mortality rates were high, modern referral centers report mortality between 10% and 26.8% with prompt care. However, the presence of peritonitis, sepsis, or necrosis of the stomach cardia remains a guarded indicator.
Even with successful surgery, complications like postoperative cardiac arrhythmias or the need for a splenectomy can lower survival chances. The condition is not a geriatric disease, with risk plateauing after the first 2–4 years for large dogs, meaning it can affect dogs of various ages.
