Pancreatitis is inflammation of the pancreas caused by the premature activation of digestive enzymes within the organ itself, leading to autodigestion and a cascade of tissue damage. While it can range from mild to fatal, severe cases often result in organ failure and require immediate veterinary intervention.

The mechanism of autodigestion

The pancreas has two main sections: an endocrine portion that produces hormones like insulin, and a larger exocrine portion made up of 82% acinar cells that produce digestive enzymes. In a healthy state, these enzymes are stored as inactive precursors called zymogens. In pancreatitis, these zymogens activate prematurely inside the pancreas instead of waiting to reach the intestine.

Once activated, the enzymes begin digesting the pancreatic tissue, the surrounding fat, and eventually the peritoneal cavity and blood circulation. This triggers a severe inflammatory response involving neutrophils and macrophages, causing interstitial oedema, haemorrhage, and necrosis of the acinar cells. The exact trigger for this disruption remains unknown in the majority of cases.

What triggers the inflammation

The specific cause is unknown for most dogs and cats, though genetic and environmental factors play a role. In Miniature Schnauzers, hypertriglyceridaemia (high blood fats) and specific gene mutations are suspected causes, though this does not appear to apply to all breeds. Other known triggers include trauma, toxins, and certain medications.

Several drugs are known to cause iatrogenic pancreatitis, including potassium bromide, phenobarbital, L-asparaginase, azathioprine, and meglumine antimonate. While glucocorticoids were once thought to be a major risk factor, current understanding has discarded this link. In cats, causes can include abdominal trauma, hypercalcaemia, organophosphate toxicity, and pancreatic tumours.

Recognizing the clinical signs

Clinical symptoms vary widely, from asymptomatic cases to life-threatening systemic failure. The most common signs in dogs are lethargy (88%), anorexia (86%), vomiting (83%), and abdominal pain (59%). Cats often present with similar signs, including dehydration, weight loss, and icterus (jaundice).

Severe cases can lead to acute kidney injury, cardiovascular shock, and disseminated intravascular coagulation, a condition where the body's clotting factors are depleted, leading to spontaneous bleeding. Dehydration is observed in nearly all dogs with severe acute pancreatitis. The inflammation can also compress the bile duct, causing extrahepatic biliary duct obstruction and jaundice.

Who is at risk

Pancreatitis can occur at any age but is most common in middle-aged to elderly animals. Obesity and diets high in fat are significant risk factors for both acute and chronic forms. Certain breeds are predisposed, including Miniature Schnauzers, Cocker Spaniels, and various terriers.

While Miniature Schnauzers are prone to the condition due to hyperlipidemia, Yorkshire Terriers appear at higher risk for the acute form, whereas Labrador Retrievers and Miniature Poodles seem to have a decreased risk. Underlying conditions like diabetes mellitus, Cushing's disease, and hypothyroidism may also increase susceptibility, though the evidence for some links is not definitive.

Diagnosis and testing

Diagnosing pancreatitis is challenging because standard blood tests like complete blood counts and serum biochemistry are not specific enough to confirm the condition on their own. The most effective diagnostic tool is the measurement of pancreatic lipase immunoreactivity, which has high sensitivity and accuracy.

Tests measuring serum amylase and lipase activities are generally not useful, as these enzymes can be produced by non-pancreatic tissues and cannot distinguish the source. Radiographs often show no abnormalities, serving mainly to rule out other conditions. Ultrasound is currently the best method for diagnosis, typically revealing hypoechoic areas within the pancreas and increased echogenicity of the surrounding mesentery.

Management and treatment

Because the root cause is often unknown, treatment is supportive and focuses on managing symptoms. The historical practice of withholding food has been abandoned; current guidelines recommend early enteral feeding, preferably via jejunal feeding tubes, to maintain gut function and reduce complications.

Pain management is critical, as the pain itself can cause further complications like anorexia and decreased blood flow to abdominal organs. Opioids such as buprenorphine, fentanyl, and hydromorphone are typically required. Antiemetics like maropitant are the drug of choice to control vomiting and nausea. In severe cases, intravenous fluid therapy is essential to correct dehydration and hypovolaemia, with colloid fluids used if the patient has low albumin or low blood pressure.

Long-term outlook

Post-pancreatitis management involves a strict low-fat diet and avoiding drugs known to trigger the condition. Some patients may require supplemental pancreatic enzymes if the acinar cells are permanently damaged, leading to exocrine pancreatic insufficiency. Damage to the endocrine portion can also result in diabetes mellitus, which may be transient or permanent depending on the severity of the initial attack.

While the prognosis varies, severe pancreatitis remains a life-threatening condition. Early recognition and aggressive supportive care significantly improve the chances of survival and recovery.
