Vomiting yellow bile usually means the dog's stomach is empty and bile has refluxed from the small intestine, but in some cases it signals a life-threatening gallbladder mucocele. This fluid is bile, a digestive fluid produced by the liver and stored in the gallbladder, which normally flows into the small intestine to break down fats. When a dog vomits on an empty stomach, the stomach has nothing to digest, so the bile backs up and is expelled, appearing yellow or greenish.
Empty stomach reflux is the common cause
This is the most frequent reason for yellow vomit and often happens after a long gap between meals. The stomach lining becomes irritated by the bile, triggering the vomiting reflex. It is a mechanical issue where the stomach empties completely, allowing bile to accumulate and irritate the gastric lining. In these cases, the dog is otherwise bright, eating well once fed, and the vomiting is an isolated event rather than a sign of systemic illness.
Gallbladder mucocele is the dangerous exception
While often benign, yellow vomiting can be a primary sign of a gallbladder mucocele, a condition where thick, gelatinous mucus accumulates in the gallbladder. This disease involves an excessive and abnormal accumulation of mucus that causes the gallbladder to enlarge, often appearing as a "kiwi-like" mass on ultrasound. The mucus is typically pale yellow to dark green, matching the color of the vomit, and can lead to a blockage of the bile ducts. Without treatment, the pressure from this thick material increases the risk of a gallbladder rupture, which is fatal.
Why certain dogs are at higher risk
This condition is most prevalent in mid-age to older dogs, with a median age of 9 years, and shows no sex predilection. Smaller breeds such as Cocker Spaniels, Miniature Schnauzers, Pomeranians, and Shetland Sheepdogs are particularly predisposed due to a mutation in the ABCB4 gene. This genetic factor often triggers lipid metabolism problems that encourage the formation of the mucus. Other underlying conditions that can lead to this issue include Cushing's disease, high cholesterol, pancreatitis, and inflammatory bowel disease.
Recognizing the warning signs beyond vomiting
About 77% of dogs with a gallbladder mucocele display vague clinical signs that can last up to a week, making early detection difficult. Alongside vomiting, owners may notice loss of appetite, lethargy, abdominal pain, jaundice, and excessive thirst or urination. The presence of these symptoms, combined with the yellow vomit, suggests the bile flow is obstructed, causing liver enzymes to rise in the blood. In severe cases, the dog may collapse from septic shock if the gallbladder ruptures and bile leaks into the abdominal cavity.
Diagnosis requires specific imaging
Diagnosis relies heavily on an ultrasound examination, which is the only effective and accurate technique for identifying this condition. The ultrasound will reveal a thickened gallbladder wall and an enlarged organ filled with immobile, echogenic bile that does not shift with gravity. Striated or stellate patterns surrounding a small amount of stationary bile are also clear indicators. Blood tests often show elevated liver enzymes, and a complete blood count may reveal an infection if white blood cell counts are high.
Medical management versus surgical intervention
Medical management is an option for asymptomatic cases without signs of rupture, involving a 6 to 8-week course of antibiotics and choleretic drugs like Ursodiol to promote bile flow. However, surgical removal of the gallbladder, known as a cholecystectomy, is recommended upon initial detection to prevent spontaneous rupture, as the rate of leakage is unpredictable. If the gallbladder has already ruptured, immediate emergency surgery is necessary to flush the abdominal cavity and drain the bile.
Recovery and long-term outlook
Post-surgical care involves hospitalization for up to 48 hours to monitor hydration and appetite, followed by a restricted exercise regime for 14 days. A low-fat diet is often recommended to reduce future complications, and the incision must be checked daily to prevent infection. While the mortality rate for surgery ranges between 22 and 32%, dogs that successfully complete the procedure show excellent long-term survival rates.
